1-   You may order different packages, options, and/or add-ons from each or just one of the proofs available. You will find your proofs on the order form we sent home with your child. The name of the proofs appear underneath each image and will coincide with the names on the envelope area of your form. We have provided examples to show you how to complete your order below.

2-   Please return this form with a check or cash payment.  Exact amounts please, no change will be provided.  

Make checks payable to MONKEY CHICKEN STUDIOS, then write your child’s name and code on the memo. The code appears on the circular area next to your proofs.
